Advance Decision

Arken Professional will also allow you to create an Advance Decision for your clients, you can select this in the document type menu when creating a document.

The drafting of this Document will function very similarly to a Will document. You need to answer all outstanding questions, and you can also mirror documents between spouses and existing documents from your client.

This document will allow you to outline the following scenarios:

1. General Advance Decision
   
2. Advance Decision defining intolerable condition
   
3. Advance Decision requesting maximum treatment
   
4. Advance Decision defining treatment preferences
   

You can also outline other medical details for your client:

1. Number of medical practicioners requires for a decision
   
2. Next of Kin
   
3. Information recorded in respect of medical care
   
4. Mediacl Practitioners that should be involved in care
   
5. Organ Donation
   
6. Further directions
   

Once all mandatory fields have been completed, you will be able to download the document.
